Device Protection Terms and Conditions

1. Provider and Governing Terms

US Mobile, Inc. ("US Mobile", "USM", "we", "our") is a reseller of optional Device Protection plans administered by AKKO LLC ("AKKO"). Device Protection plans are optional. Should you choose to add Device Protection, you agree to all terms and conditions, including AKKO's Terms of Use, privacy policy, and disclosures found here.

2. Covered Losses

Insurance: Loss and theft
Service Contract: Accidental damage or breakdown (incl. electrical or mechanical failure)

3. Covered Equipment

Device Protection plans are available for phones (includes wireless device and battery). Coverage applies to only one line at any given time and the covered device will be the device that, at the time of loss, has been activated, enrolled and verified (or re-verified) in accordance with the provisions of Sections 4 and 10 below. You must own or lease the device and must use the device (logged voice or data use) on your enrolled wireless line after initial enrollment.
Exclusions: tablets, wearables, laptops, or other IoT devices.

4. Plan Activation

Device Protection plans are not active until all enrollment steps are completed, including activating the associated US Mobile line and completing the AKKO enrollment process, which includes providing device information and verifying device condition where required.
Coverage for damage, loss, and theft becomes effective immediately once all enrollment steps are completed. Mechanical and electrical breakdown coverage becomes effective 30 days after the Device Protection plan activation date.

5. Pricing and Taxes

Pricing and promotions for Device Protection plans are subject to change at US Mobile's discretion. Plan prices exclude sales tax. Sales tax will be calculated separately and applied to the taxable service contract portion of the plan price. The total tax amount varies by location.

6. Payment and Autopay

Enrollment in this Plan will continue to renew monthly (or every 12 months, if you purchased the annual Plan) on your US Mobile account until cancelled. If Autopay is enabled, US Mobile will attempt to collect payment up to 2 days before the due date, on the due date, and up to 2 days after the due date.
If payment is not successfully processed by the due date, your Device Protection plan will enter "Past Due" status, during which time new claims cannot be filed. You may restore your plan to active status by making payment within 30 days. If payment is not made within 30 days after the due date, the Device Protection plan will be canceled.

7. Cancellation

Cancellation may be requested at any time by contacting US Mobile support. US Mobile and AKKO reserve the right to cancel plans for suspected abuse, fraud, material misrepresentation, or other violations of these Terms. Any refund due upon cancellation will be processed according to the refund policy below.

8. Refund Policy

US Mobile will issue a full refund if you cancel within 30 days of activation. After 30 days, the following refund policy applies:
Annual Plan: Refunds will be calculated on a pro-rated basis in accordance with the 360-day plan duration.
Monthly Plan: No refunds will be issued; however, no further charges will be incurred. Claims may continue to be submitted through AKKO until the conclusion of the current plan cycle.

9. Communications

By purchasing a Device Protection plan, users consent to receive all communications electronically, including via email and text message. No paper mail will be sent.

10. Device Changes

It is Your responsibility to ensure that US Mobile and Akko always have the most up-to-date device information. If you change devices, You must complete the enrollment and verification steps in Section 4 to ensure your new device is associated with your active line under the Plan. While US Mobile will attempt to update your profile when we receive notification from the carrier about a device change, carrier information may be delayed or updates may not process successfully. Further, profile updates are not a substitute for completing the enrollment steps in Section 4. Claims can only be filed if the new device is associated with the line that is active on the plan, after following the enrollment and verification procedures in Section 4. US Mobile is not responsible for damage, theft or loss for devices not enrolled and verified in accordance with Section 4.

11. Modifications

US Mobile and AKKO reserve the right to modify these Device Protection terms at any time. Material changes will be communicated in accordance with applicable law.
