If you are not in New York State, don’t worry. T-Mobile will require additional documentation to process your request. After you’ve contacted T-Mobile, provide any one of the following documents:

  • Signed affidavit from a licensed medical or mental healthcare provider; victim services provider, or a court employee acting within their scope of employment
  • Police report or police statements provided to Magistrates/judges
  • Charging documents
  • Court orders
  • Protective or restraining orders
  • Any other official record that documents the domestic violence
